Indications

  • Control of severe or incapacitating allergic conditions
  • Treatment of collagen diseases like lupus erythematosus and rheumatoid arthritis
  • Management of dermatologic diseases such as bullous dermatitis herpetiformis and pemphigus
  • Treatment for primary or secondary adrenocortical insufficiency
  • Management of gastrointestinal diseases like regional enteritis and ulcerative colitis
  • Treatment for leukemias and lymphomas
  • Management of nervous system disorders including acute exacerbations of multiple sclerosis
  • Induction of diuresis or remission of proteinuria in renal diseases
  • Use in respiratory diseases like berylliosis and fulminating pulmonary tuberculosis
  • Adjunctive therapy for short-term administration in acute gouty arthritis and ankylosing spondylitis
  • Diagnostic testing of adrenocortical hyperfunction
  • Treatment for trichinosis with neurologic or myocardial involvement

Pharmacology

Decreases inflammation by inhibiting the migration of leukocytes and reversal of increased capillary permeability, suppresses normal immune response

Dosage & Administration

  • Oral dosages in adults from 1 mg to 10 mg
  • Oral dosages in children from 0.03-0.20 mg/kg body weight
  • Intravenous, intramuscular, or local injection administration
  • Use of infusion fluids like Sodium chloride 0.9%, Anhydrous glucose 5%, Invert sugar 10%, Sorbitol 5%, Ringer's solution
  • Doses of 0.05-0.20 mg/kg body weight for systemic therapy in adults
  • Recommended local therapy doses for intra-articular, intrabursal, and tendon sheath injections

Interaction

  • Enhanced potassium loss with diuretics and/or cardiac glycosides
  • Potential impairment of glucose tolerance with antidiabetics
  • Increased incidence and/or severity of gastrointestinal ulceration with non-steroidal anti-inflammatory drugs
  • Alteration of the need for oral anti-coagulants
  • Decreased effectiveness when used concomitantly with liver enzyme inducing drugs
  • Potential reduction in glucocorticoid dosage caution with concomitant use of salicylates
  • Impairment of gastrointestinal absorption when used concomitantly with antacids

Contraindications

  • Gastric and duodenal ulcers
  • Systemic and ophthalmic fungal infections
  • Viral infections like varicella and herpes genitalis infections
  • Glaucoma
  • Hypersensitivity to corticosteroids

Side Effects

  • Cushing-like syndrome
  • Fluid and electrolyte disturbances
  • Musculo-skeletal effects like myopathy and osteoporosis
  • Gastro-intestinal effects including ulceration and perforation
  • Dermatological effects like impaired wound healing and skin atrophy
  • Central nervous system effects such as psychic disturbances and convulsions
  • Immunosuppressive effects leading to increased susceptibility to infections

Pregnancy & Lactation

Pregnancy Category C, should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Glucocorticoids appear in breast milk, mothers taking high dosages should be advised not to breast-feed.

Precautions & Warnings

  • Use the lowest possible dose to control the condition under treatment
  • Use with caution in patients with congestive heart failure, hypertension, or renal insufficiency
  • Minimize drug-induced secondary adrenocortical insufficiency by gradual reduction of dosage
  • Use with caution in active or latent peptic ulcers, diverticulitis, and nonspecific ulcerative colitis
  • Special consideration for patients at increased risk of osteoporosis before initiating corticosteroid therapy
  • Observe for acute myopathy with high doses, especially in patients receiving concomitant therapy with neuromuscular blocking drugs

Use in Special Populations

To minimize potential growth effects, pediatric patients should be titrated to the lowest effective dose

Overdose Effects

Treatment is by supportive and symptomatic therapy
