- Pharmacological Action:
- Dopamine antagonist
- Gastroprokinetic
- Blocks dopamine receptors in Chemoreceptor Trigger Zone and stomach
- Restores normal motility and tone of the upper gastrointestinal tract
- Facilitates gastric emptying
- Enhances antral and duodenal peristalsis
- Regulates contraction of the pylorus
- Increases esophageal peristalsis
- Regulates lower esophageal sphincter pressure
- Usual Adult Dose: 10-20 mg (1-2 tablets or 10-20 ml suspension) every 6-8 hours daily, maximum 80 mg daily
- Usual Children Dose: 2-4 ml suspension/10 kg body weight or 0.4-0.8 ml pediatric drops/10 kg body weight every 6-8 hours daily
- Route of Administration: Oral, rectal in suppositories
- Interaction:
- Anticholinergic drugs may antagonize the antidyspeptic effect
- Antacids and antisecretory drugs lower its oral bioavailability
- Metabolized through CYP3A4 pathway, may be affected by inhibitors
- No influence on neuroleptics
- Suppression of unwanted peripheral effects of dopaminergic agonists
- Contraindications:
- Known hypersensitivity
- Neonates
- Gastrointestinal bleeding
- Mechanical obstruction or perforation
- Prolactinoma
- Side Effects:
- Transient intestinal cramps
- Rare extrapyramidal phenomena in children and adults
- Potential increase in plasma prolactin level
- Rare allergic reactions like rash and urticaria

- Use in the first trimester justified by therapeutic benefit
