Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
Mechanism of Action
- Inhibition of PDE5 by Sildenafil produces increased levels of cGMP, resulting in smooth muscle relaxation and inflow of blood into the penile tissues, thereby producing an erection
Pharmacokinetics and Metabolism
- Rapid absorption after oral administration
- Mean absolute bioavailability of 41%
- Eliminated predominantly by hepatic metabolism
- Converted to an active metabolite with similar properties
Dosage & Administration
- Recommended starting dose of 50 mg
- May be increased to a maximum recommended dose of 100 mg
- Maximum recommended dosing frequency is once per day
Interaction
- Metabolism primarily mediated by cytochrome P450 isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce clearance while inducers may increase clearance
Contraindications
- Hypersensitivity to any component
- Use with organic nitrates

- Digestive: vomiting, gastritis, liver function tests abnormal
- Hemic and Lymphatic: anemia, leukopenia
- Metabolic and Nutritional: edema, hyperglycemia, hypoglycemic reaction
- Musculoskeletal: arthritis, myalgia, synovitis
- Nervous: ataxia, depression, insomnia
- Respiratory: dyspnea, pharyngitis, sputum increased
- Skin and Appendages: urticaria, sweating, exfoliative dermatitis
- Special Senses: sudden decrease or loss of hearing, mydriasis, tinnitus
- Urogenital: cystitis, urinary frequency, abnormal ejaculation, anorgasmia
Pregnancy & Lactation
- Pregnancy category B
- Not indicated for use by women
- No evidence of teratogenicity or embryotoxicity in animal studies
Precautions & Warnings
- Caution advised when co-administered with alpha-blockers
- Consideration for patients with underlying cardiovascular disease
- Sensitivity to actions of vasodilators
- Careful use in patients with certain medical conditions
Overdose Effects
