Indications
- Relief of signs and symptoms of osteoarthritis
- Relief of signs and symptoms of rheumatoid arthritis
- Relief of signs and symptoms of ankylosing spondylitis
- Decrease the risk of developing gastric ulcers in patients at risk of NSAID-associated gastric ulcers
Pharmacology
- Immediate release Esomeprazole Magnesium layer
- Enteric-coated Naproxen core
- Inhibition of prostaglandin synthesis by Naproxen
- Suppression of gastric acid secretion by Esomeprazole
Dosage
- Use lowest effective dose
- Adult dosage: 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole twice daily
- Adolescent patient dosage: 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole
Administration
- Tablet should not be split, chewed, crushed or dissolved
- To be taken at least 30 minutes before meals
Interaction
- Concomitant use of NSAIDs with ACE inhibitors, diuretics & beta-blockers
- Concomitant use with warfarin
- Inhibition of gastric acid secretion by Esomeprazole affecting drug absorption
- Effect of high-fat food on absorption of Naproxen & Esomeprazole
Contraindications
- Known hypersensitivity to any component of the tablet or substituted benzimidazoles
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Peri-operative use in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Decrease in incidence of gastrointestinal side effects from Naproxen
- Decrease in occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events compared to Naproxen alone
- Potential risk of arterial thrombotic events associated with high doses and long-term use of NSAIDs
- Adverse events such as oedema, hypertension, and cardiac failure reported in association with NSAID treatment
Pregnancy & Lactation
- Use not recommended during first and second trimester of pregnancy
- Use contraindicated during the third trimester of pregnancy
- Unknown excretion of esomeprazole in human milk
- NSAID use may impair female fertility
Precautions & Warnings
- Avoid the combination of Naproxen with other NSAIDs
- Minimize undesirable effects by using lowest effective dose for shortest duration
- Increased risk of adverse events in older people
- Potential impact on vitamin absorption and gastrointestinal infections with long-term use of proton pump inhibitors
Use in Special Populations
- Increased unbound plasma fraction of naproxen in elderly patients
- Not recommended for use in patients with moderate to severe renal impairment
- Close monitoring and possible dose reduction in patients with mild to moderate hepatic impairment
