Pharmacology
- Inhibition of CNS neuronal uptake of serotonin
- Enhancement of actions of serotonin on 5HT1A autoreceptors
- Virtually no affinity for certain receptors
Dosage & Administration
- Recommended starting dose for adults: 50 mg
- Titration within a dose range of 100 to 300 mg/day for adult patients
- Recommended starting dose for pediatric populations (ages 8-17 years): 25 mg
- Titration within a dose range of 50 to 200 mg/day for pediatric patients
- Dose adjustment in adolescents may be indicated to achieve therapeutic benefit
- Dose modification for elderly or hepatically impaired patients
Interaction
- Inhibition of cytochrome P450 isoenzymes
- Elevated levels and symptoms of toxicity with coadministration of carbamazepine
- Rare postmarketing reports of weakness, hyperreflexia, and incoordination with sumatriptan
- Increased tacrine C max and AUC with coadministration of tacrine
- Significantly increased plasma TCA levels with coadministration of tricyclic antidepressants (TCAs)
- Severe vomiting with coadministration of tryptophan
- Bradycardia with coadministration of diltiazem
- Dose reduction and cautious titration when coadministered with propranolol or metoprolol
Contraindications
- Coadministration of tizanidine, thioridazine, alosetron, pimozide
- Serotonin Syndrome and MAOIs
- Do not use within 14 days of stopping an MAOI intended to treat psychiatric disorders
- Do not start in a patient being treated with linezolid or intravenous methylene blue
