Indications
- Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus
- Reduce the risk of cardiovascular death in adult patients with type 2 diabetes mellitus and established cardiovascular disease
Pharmacology
- Sodium glucose co-transporter-2 (SGLT-2) inhibitor
- Reduces renal absorption of glucose
- Leads to increased glucose excretion
- Contributes to reduced hyperglycemia, weight loss, and reduced blood pressure
Dosage & Administration
- Recommended dose of 10 mg once daily
- Can be taken with or without food
- Dose may be increased to 25 mg once daily in patients tolerating it
Interaction
- Co-administration with diuretics increases urine volume
- Increased risk of hypoglycemia when co-administered with insulin or insulin secretagogues
- SGLT2 inhibitors lead to positive urine glucose tests and unreliable 1,5-AG assay measurements for glycemic control
Contraindications
- Not for patients with serious hypersensitivity reaction
- Not for patients with severe renal impairment, end-stage renal disease, or undergoing dialysis
Side Effects
- Common adverse reactions: urinary tract infections and female genital mycotic infections
- Other common side effects: dehydration, hypotension, weakness, dizziness, increased thirstiness
Pregnancy & Lactation
- Potential benefit justifies the potential risk to the fetus for use during pregnancy
- Not recommended during breastfeeding
Precautions & Warnings
- Assessment of renal function recommended prior to initiation
- Not initiated in patients with an eGFR less than 45 ml/min/1.73 m^2
- No dose adjustment needed in patients with eGFR greater than or equal to 45 ml/min/1.73 m^2
Overdose Effects
- Usual supportive measures recommended in the event of an overdose
- Hemodialysis has not been studied for Empastar removal
