Indications
- Treatment of chronic iron overload due to repeated blood transfusions
- Treatment for thalassemia, sickle cell disease, myelodysplastic syndrome
- Treatment for non-transfusion-dependent thalassemia syndromes
Pharmacology
- Orally active chelator
- Highly selective for iron
- Binds iron in a 2:1 ratio
- Promotes excretion of iron in faeces
- Low affinity for zinc and copper
Dosage
- Initial daily dose of 20-40 mg/kg body weight
- Recommended to be taken on an empty stomach
- Dispersion in water or juice
- Monitoring serum ferritin levels
- Possible dose adjustments every 3 to 6 months
Administration
- Taken once daily on an empty stomach
- Tablets dispersed in water or juice
- Not to be chewed or swallowed whole
- Avoid dispersion in carbonated drinks or milk
Interaction
- Not to be taken with aluminium-containing antacids
- Caution with drugs metabolized through CYP3A4
- Possible dose adjustments with concomitant use of certain drugs
Contraindications
- Hypersensitivity to the active substance or excipients
- Creatinine clearance less than 40 ml/min
- Serum creatinine >2 times the upper limit of normal

- Cytopenias
- Gastrointestinal symptoms
- Headache and dizziness
- Elevation of hepatic transaminases
- Rare cases of skin rash and Stevens-Johnson syndrome
- Auditory and ocular disturbances

- Pregnancy Category C
- Potential risk to the fetus
- Unknown excretion in human milk
Precautions & Warnings
- Risk of gastrointestinal bleeding and ulceration
- Cytopenias and serious hypersensitivity reactions
- Caution with drugs with ulcerogenic or hemorrhagic potential
Use in Special Populations
- Similar safety and efficacy in pediatric patients
- Increased risk in elderly patients
- Not studied in patients with renal or hepatic impairment
