Indications
- Reduces risk of cardiovascular death
- Treats symptomatic heart failure

Pharmacology
- Neprilysin inhibitor
- Angiotensin receptor blocker
- Inhibits neprilysin
- Blocks angiotensin II receptor
- Effects attributed to increased levels of peptides
Dosage & Administration
- Adult starting dose: 49/51 mg orally twice daily
- Pediatric dose adjustment every 2 weeks
- Recommended dose titration for pediatric patients
Interaction
- Dual blockade contraindicated
- Potassium-sparing diuretics increase serum potassium level
- NSAIDs increase risk of renal impairment
- Lithium increases risk of toxicity
Contraindications
- Hypersensitivity to any component
- History of angioedema related to previous ACE inhibitor or ARB therapy
- Concomitant use of ACE inhibitors and aliskiren in diabetes patients

Pregnancy & Lactation
- Safety not established in pediatric patients less than 1 year

- May cause angioedema
- Lowers blood pressure and may cause hypotension
- Monitor serum potassium periodically
- Dosage reduction or interruption may be required
