Indications

Pharmacology
- Proton pump inhibitor
- Suppresses gastric acid secretion
- Specific inhibition of the H+/K+-ATPase
- Enteric-coated pellet formulation
- Peak plasma levels at approximately 1.5 hours
- Metabolized in the liver by CYP enzymes
- Low urinary excretion
Dosage
- Oral dosage form for healing and maintenance of erosive esophagitis, symptomatic GERD, Helicobacter pylori eradication, Zollinger-Ellison Syndrome, acid-related Dyspepsia, duodenal and gastric ulcer
- IV injection or infusion for GERD with erosive esophagitis and risk reduction of rebleeding of gastric or duodenal ulcers
Administration
- Tablet or capsule should be swallowed whole one hour before a meal
- Oral suspension should be taken with water
- IV injection solution must be administered intravenously over at least 3 minutes
Interactions
- Extensively metabolized by CYP2C19 and CYP3A4
- No inhibition of CYPs 1A2, 2A6, 2C9, 2D6, 2E1, and 3A4
- No clinically relevant interactions with specific drugs
- Potential interference with drugs affected by gastric pH
- Increased plasma levels when combined with certain drugs
Contraindications
- Known hypersensitivity to any of the formulation

Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- No teratogenic effects in animal studies

- Symptomatic response does not preclude the presence of gastric malignancy
- Advice for patients on capsule administration and use of antacids
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- No overall differences in safety and efficacy in the elderly
- No dosage adjustment required for mild to moderate hepatic insufficiency
- No alteration in pharmacokinetics in renal impairment
Overdose Effects
- Lethal in high doses in rat studies
- Symptomatic and supportive treatment recommended
- Not expected to be removed by dialysis
