Indications
- Treatment of anemia in Chronic Kidney Disease (CKD) patients on dialysis and not on dialysis
- Treatment of anemia in Zidovudine-treated HIV-infected patients
- Treatment of anemia in patients undergoing myelosuppressive chemotherapy
- Reduction of allogeneic RBC transfusions in patients undergoing elective, noncardiac, nonvascular surgery
- Prevention of anemia of prematurity in infants with a birth weight of 750 to 1500 gm and gestational age of less than 34 weeks
Description
- Recombinant human erythropoietin (EPO)
- Expressed in Chinese hamster ovary cells
- 165 amino acid sequence identical to human urinary EPO
- Apparent molecular weight of erythropoietin is about 30,400 daltons
Dosage
- Regular monitoring of hemoglobin levels
- Evaluation of iron stores and nutritional factors before and during treatment
- Adjustment of therapy based on hemoglobin rate of rise, rate of decline, ESA responsiveness, and hemoglobin variability
- Different dosing guidelines for CKD patients on dialysis and not on dialysis, Zidovudine-treated HIV-infected patients, and cancer chemotherapy patients
Administration
- Preparation and administration precautions
- Do not shake the product
- Protect syringe from light
- Visual inspection for particulate matter and discoloration prior to administration
- Discard unused portions of Erythropoietin alfa
Interaction
- Potentiation by simultaneous therapeutic administration of a haematinic agent like ferrous sulphate
Contraindications
- Uncontrolled hypertension
- Serious allergic reactions to Erythropoietin alfa
- History of Pure Red Cell Aplasia (PRCA) following treatment with any erythropoietin
Side Effects
- Adverse reactions reported in patients with CKD, Zidovudine-treated HIV-infected Patients, cancer patients on chemotherapy, surgery patients, and premature infants
Pregnancy & Lactation
- Limited data in pregnant women
- Reproduction toxicology observed in animals
- Use in pregnancy only if potential benefit outweighs potential risk to the fetus
- Caution advised in nursing women
Overdose Effects
- Wide therapeutic margin
- Can cause hemoglobin levels above the desired range
- Management with discontinuation or reduction of dosage and/or phlebotomy
- Severe hypertension observed in cases of overdose

- Store at 2°C to 8°C
- Do not freeze or shake
- Maintain temperature range until administration
- Store in original package to protect from light
