Description
Makmaul Sanagra is a selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5).
Mode of Action
- Makmaul Sanagra produces increased levels of cGMP in the corpus cavernosum, resulting in smooth muscle relaxation and inflow of blood into the penile tissues, thereby producing an erection.
Pharmacology
- Mechanism of Action: Involves release of nitric oxide, activation of guanylate cyclase, and increased levels of cGMP
- Pharmacokinetics and Metabolism: Rapid absorption, hepatic metabolism, terminal half lives of about 4 hours, rapid onset of action
- Absorption and Distribution: Rapidly absorbed, maximum observed plasma concentrations reached within 30 to 120 minutes
- Metabolism and Excretion: Cleared by hepatic microsomal isoenzymes, excreted as metabolites predominantly in the feces
- Pharmacokinetics in Special Populations: Reduced clearance in healthy elderly volunteers
Dosage & Administration
- Erectile dysfunction: Recommended dose of 50 mg taken approximately 1 hour before sexual activity, with a maximum recommended dosing frequency of once per day
- Pulmonary arterial hypertension: Recommended dose of 20 mg three times a day, approximately 4-6 hours apart, with or without food
Interaction
- Metabolism: Principally mediated by the cytochrome P450 (CYP) isoforms 3A4 and 2C9, leading to potential drug interactions
- Factors affecting plasma levels: Age >65, hepatic impairment, severe renal impairment, and concomitant use of potent CYP3A4 inhibitors
Contraindications
- Hypersensitivity to any component of the medication
- Concurrent use of organic nitrates
