Indications

Pharmacology
- Active form: Calcitriol
- Binding to Vitamin D receptors (VDRs)
- Fat-soluble with a half-life of about 50 days
- Absorption in the small intestine
- Metabolism in the liver and kidney
- Increase in calcium absorption
Dosage and administration
- Adult dosage: 40000 IU once weekly for 7 weeks for Vitamin D3 deficiency
- Children dosage: 20000 IU once every 2 weeks for 6 weeks for Vitamin D3 deficiency
- Various dosage forms: capsule, film-coated tablet, oroflash/chewable tablets, syrup, injection
Interaction
- Interference with absorption and metabolism by various drugs
- Increased need for Vitamin D3 with corticosteroids
Contraindications
- Hypercalcaemia
- Hypersensitivity to Vitamin D3 or excipients
- Evidence of Vitamin D3 toxicity
Side effects
- Anorexia
- Nausea & vomiting
- Diarrhea
- Constipation
- Weight loss
- Headache
- Sweating
- Vertigo
- Raised concentrations of calcium and phosphate in plasma and urine
Pregnancy and lactation
- Safe use up to 4000 IU during pregnancy
- Excretion in breast milk
- Need for higher dose in Vitamin D3 deficient pregnant women
- Consideration of additional dose for breast-fed infants
Precautions and warnings
- Monitoring plasma-calcium concentration
- Consultation for high doses in renal impairment and pregnancy
- Consultation for supplementing with Digoxin and Thiazide Diuretics
- Consultation for liver or kidney disease, primary hyperthyroidism, lymphoma, tuberculosis, and granulomatous disease

- Safety & efficacy not established in children under 12 years
