Indications
- Anaphylaxis, asthma, severe hypersensitivity reactions
- Rheumatoid arthritis, juvenile chronic arthritis, polymyalgia rheumatica
- Systemic lupus erythematosus, dermatomyositis, mixed connective tissue disease
- Polyartritis nodosa, sarcoidosis
- Pemphigus, bullous pemphigoid, pyoderma gangrenosum
- Minimal change nephrotic syndrome, acute interstitial nephritis
- Rheumatic carditis
- Ulcerative colitis, Crohn's disease
- Uveitis, optic neuritis
- Autoimmune hemolytic anemia, idiopathic thrombocytopenic purpura
- Acute and lymphatic leukemia, malignant lymphoma, multiple myeloma
- Immune suppression in transplantation
Pharmacology
- Anti-inflammatory action by inhibiting Phospholipase A2 enzyme
- Decreases release of certain chemicals in the immune system
- Immunosuppressive action
Dosage & Administration
- Adults: Up to 120 mg/day for acute disorders
- Maintenance doses in most conditions are within the range of 3-18 mg/day
- Children: Doses usually lie in the range 0.25-1.5 mg/kg/day
- Deflazacort withdrawal
- Maintenance dose for juvenile chronic arthritis: 0.25-1.0 mg/kg/day
Interaction
- Metabolized in the liver
- Recommended to increase maintenance dose if liver enzyme inducers are co-administered
- Possible to reduce maintenance dose for drugs which inhibit liver enzymes
Contraindications
- Hypersensitivity to any of the ingredients
- Patients receiving live virus immunization
Side Effects
- GI disturbances
- Musculoskeletal, endocrine, neuropsychiatric, ophthalmic effects
- Fluid and electrolyte disturbances
- Susceptible to infection, impaired healing
- Hypersensitivity, skin atrophy, striae, telangiectasia, acne
Pregnancy & Lactation
- Corticosteroids may increase the risk of intrauterine growth retardation
- Corticosteroids are excreted in breast milk
Precautions & Warnings
- Special caution required for cardiac disease, congestive heart failure, hypertension
- Need for frequent patient monitoring
- Salt and water retention, increased excretion of potassium
- Dietary salt restriction and potassium supplementation may be necessary
- Need for caution in gastritis, oesophagitis, diverticulitis, ulcerative colitis
- Special caution in diabetes mellitus, family history, osteoporosis, myasthenia gravis
Use in Special Populations
- Hepatic Impairment: Careful monitoring and adjustment of the minimum effective dose
- Renal Impairment: No special precautions necessary
- Elderly: No special precautions necessary
