Indications

  • Essential hypertension
  • Angina pectoris
  • Vasospastic angina

Pharmacology

  • Dihydropyridine calcium-channel blocker
  • Long duration of action
  • Vasodilation
  • Reduced peripheral resistance
  • Increased coronary blood flow

Dosage & Administration

  • Usual dose: 5 mg once daily for hypertension
  • Max dose: 10 mg once daily for hypertension
  • 5 to 10 mg for chronic stable or vasospastic angina
  • May be taken without regard to meals

Interaction

  • Potentially hazardous interactions with p-blockers
  • No interaction between Xelcard and Digoxin
  • No interaction between Xelcard and Cimetidine
  • No significant alteration of Warfarin's effect by Xelcard
  • Food does not alter the absorption of Xelcard

Contraindications

  • Hypersensitivity to dihydropyridine derivatives
  • Pregnancy

  • Increased micturition frequency
  • Lethargy
  • Eye pain

Pregnancy & Lactation

  • Pregnancy Category C
  • Use during pregnancy only if benefit justifies risk
  • Not known whether excreted in human milk
  • Discontinue nursing while Xelcard is administered

  • Hepatic impairment
  • Pregnancy
  • Breast feeding

Use in Special Populations

  • Different doses for children with hypertension
  • Effect on blood pressure in children under 6 is not known
  • Equal tolerance in elderly as in younger patients
  • Normal dosage recommended for renal impairment
  • Caution in dosage for mild to moderate hepatic impairment
  • Initiate at lowest dose for severe hepatic impairment

Overdose Effects

  • Excessive peripheral vasodilatation
  • Reflex tachycardia
  • Marked systemic hypotension
  • Shock with fatal outcome
  • Active cardiovascular support necessary
  • Elevation of extremities
  • Monitoring of cardiac and respiratory function
  • Attention to circulating fluid volume and urine output
  • Use of vasoconstrictor and intravenous calcium gluconate
