Indications
- Neutralising the immediate toxic effects of folic acid antagonists, e.g. methotrexate
- Calcium folinate rescue- a treatment technique using calcium folinate in conjunction with folic acid antagonists, e.g. methotrexate, to minimise systemic toxicity
- Treatment of megaloblastic anaemias due to sprue, nutritional deficiency, pregnancy, infancy, liver disease and malabsorption syndrome
Composition
- Calcium Folinate Hydrate BP 6.249 mg equivalent to Folinic Acid 5 mg
- Calcium Folinate Hydrate BP 18.746 mg equivalent to Folinic Acid 15 mg
Pharmacology
- Active form of folic acid
- Cofactor for 1-carbon transfer reactions
- Impairment of thymidylate synthesis
- Involvement in biosynthesis of purine and pyrimidines
- Potent antidote for hematopoietic and reticuloendothelial toxic effects of folic acid antagonists
- Therapy with 'Folinate rescue'
Dosage & Administration
- Commencement of calcium folinate rescue within 24 hours after methotrexate infusion
- Administration of Folinic Acid at a dose of 15 mg every 6 hours for 10 doses for methotrexate rescue
- 5-15 mg of Folinic Acid per day for countering hematologic toxicity from folic acid antagonists with less affinity for mammalian dihydrofolate reductase
Interaction
- Counteraction of antiepileptic effect of phenobarbitone, phenytoin, and primidone by large amounts of folinates
- Caution during concurrent administration with fluoropyrimidine
Contraindications
- Hypersensitivity to calcium folinate or any other components
- Pernicious anemia or other megaloblastic anemia with deficient vitamin B12
- Galactose intolerance or glucose-galactose malabsorption

Pregnancy & Lactation
- Use in pregnant women only if potential benefit justifies potential risk
- Caution during administration to nursing mothers
Precautions & Warnings
- Use only with methotrexate or 5-FU under supervision of experienced clinician
- Early administration in case of folic acid antagonist overdosage
- Avoid simultaneous administration with methotrexate
- Measures to ensure prompt excretion of methotrexate in Calcium Folinate Rescue Therapy
