Indications
- Reversal of hypotension induced by spinal or epidural anaesthesia
Pharmacology
- Ephedrine is a stimulant drug
- Belongs to sympathomimetics group
- Alpha and beta adrenergic agonist
- Enhances release of norepinephrine
- Increases activity of central nervous system
Dosage & Administration
- Dose: 3 to 6 mg Ephedrine Hydrochloride for hypotension
- Route: Slow intravenous injection
- Repeat every 3 to 4 minutes as required
- Maximum total dose: 30 mg
- Can be given intramuscularly or subcutaneously
Interaction
- May cause hypertensive crisis in patients on MAO inhibitors
- Should be used with caution in patients on volatile anaesthetics
- Increased risk of arrhythmias in patients receiving certain medications
- Increased risk of vasoconstrictor effects in patients receiving certain medications
Contraindications
- Contraindicated in presence of coronary thrombosis
- Caution in patients with organic heart disease, hyperthyroidism, hypertension, and angina
- Caution in patients receiving digitalis
Side Effects
- Nausea, vomiting, anorexia
- Tachycardia, bradycardia, arrhythmias
- Anginal pain, hypertension, hypotension
- Dizziness, flushing
- Dyspnoea, headache, anxiety
- Restlessness, confusion, psychoses
- Insomnia, tremor
- Difficulty in micturition, urine retention
- Sweating, hypersalivation
- Changes in blood-glucose concentration
- Rarely angle-closure glaucoma
Pregnancy & Lactation
- Pregnancy Category C
- Not recommended during lactation due to risk of adverse effects in infant
Precautions & Warnings
- Caution in patients with hyperthyroidism, diabetes mellitus, ischemic heart disease, hypertension, renal impairment, or angle-closure glaucoma
- May increase difficulty with micturition in patients with prostatic enlargement
- Increased fetal heart rate reported with parenteral use
- Irritability and disturbed sleep reported in breast-fed infants
Overdose Effects
- Minor reactions in most patients
- Giddiness, headache, nausea, vomiting
- Sweating, palpitations
- Difficulty in micturition, tremors
- Anxiety, restlessness, insomnia
