Indications
- Treatment of reversible bronchospasm associated with COPD
- Treatment of reversible airway obstruction in asthma
- As bronchodilator in the treatment of chronic reversible airway obstruction in asthma and COPD including chronic bronchitis and emphysema
- Treatment of acute reversible airway obstruction
- Concomitant use with inhaled beta 2-agonists
Pharmacology
- Quaternary ammonium compound with anticholinergic properties
- Inhibition of vagal reflexes
- Antagonizes the action of acetylcholine
- Prevents increase in intracellular concentration of cyclic guanosine monophosphate
- Acts on bronchial smooth muscle
Dosage
- Adult dosage: 250-500 micrograms (1-2 ml) 3 to 4 times daily
- 500 micrograms/2 ml for acute bronchospasm
- Children 6-12 years: 250 micrograms (1 ml) up to a total daily dose of 1 mg (4 ml)
- Children 0-5 years (for treatment of acute asthma only): 125-250 micrograms (0.5 ml-1 ml) up to a total daily dose of 1 mg (4 ml)
- Usable with a range of commercially available nebulizing devices
Administration
- Proper inhaler use for effective medication delivery
- Supervised administration for children
- Instructions for cleaning inhaler
- Technique check in front of a mirror
- Rinse mouth after each treatment
Interaction
- Concomitant use with sympathomimetic bronchodilators, methylxanthines, steroids, and cromolyn sodium
- No fully evaluated interaction effects with the mentioned drugs
Contraindications
- Hypersensitivity to atropine or its derivatives
- Hypersensitivity to Ipratropium, Atropine or its derivative
- Hypersensitivity to soya lecithin or related food products
Side Effects
- Common side effects: Headache, Dizziness, Cough, Dryness of mouth, Vomiting
- Potential adverse effects: Urticaria, Tachycardia, Skin rash, Pruritus, Anaphylactic reaction, Eye pain, Nausea
- Symptomatic adverse effects: Dry mouth, Cough, Irritation from aerosol, Headache, Nausea, Dizziness, Blurred vision, Difficulty in accommodation, Nervousness, Paresthesias, Drowsiness, Coordination difficulty, Itching
- Life-threatening effects are rare
Pregnancy & Lactation
- No established safety during pregnancy
- Caution advised when administered to nursing mothers
Precautions & Warnings
- Close medical supervision during initial dosing
- Caution in patients with narrow-angle glaucoma, prostatic hyperplasia, bladder-outflow obstruction, or cystic fibrosis
- Avoid aerosol contact with the eyes
- Temporary blurring of vision or eye pain may result if aerosol is sprayed into the eyes
- Not indicated for initial treatment of acute episodes of bronchospasm where rapid response is required
Overdose Effects
- Acute overdosage by inhalation is unlikely
- Inhaled dosage of 5 mg increases heart rate and palpitation
- Single doses of 30 mg by mouth resulted in anticholinergic side effects
