Indications
- Therapy with lipid-altering agents

Pharmacology
- Atorvastatin lowers plasma cholesterol
- Inhibiting HMG-CoA reductase and cholesterol synthesis
- Increasing hepatic LDL receptors
- Ezetimibe reduces blood cholesterol
- Inhibiting absorption of cholesterol by the small intestine
- Molecular target of ezetimibe - sterol transporter NPC1L1

Dosage & Administration
- Dosage range: 10/10 mg to 80/20 mg daily
- Recommended starting dose: 10/10 mg or 20/10 mg daily
- Administration as a single dose at any time of the day
- Recommended starting dose for patients requiring larger reduction in LDL-C: 40/10 mg daily
- Dosage for patients with homozygous familial hypercholesterolemia: 40/10 mg or 80/10 mg daily
- Dosage for patients with hepatic impairment
- Dosage for patients with renal impairment
Interaction
- Risk of myopathy during treatment with statins
- Concurrent administration of fibric acid derivatives
- Interaction with lipid-modifying doses of niacin
- Concurrent administration with cyclosporine and strong CYP3A4 inhibitors
- Interaction with gemfibrozil and niacin
- Monitoring for patients taking digoxin
Contraindications
- Active liver disease
- Unexplained persistent elevations of hepatic transaminase levels
- Hypersensitivity to any component of the preparation

Pregnancy & Lactation
- Category X medication
- Contraindicated in women who are or may become pregnant
- Suggested discontinuation of lipid-lowering drugs during pregnancy
- Uncertain excretion of atorvastatin in human milk
Precautions & Warnings
- Risk of myopathy and rhabdomyolysis
- Temporary withholding or discontinuation in patients with myopathy
- Liver enzyme tests prior to initiating therapy
- Caution with drugs decreasing levels or activity of endogenous steroid hormones
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- No dosage adjustment necessary in geriatric patients
