Indications
- Treatment of chronic angina
- Decrease angina episodes
- Used with beta-blockers, nitrates, calcium channel blockers, anti-platelet therapy
- Lipid-lowering therapy
- ACE inhibitors
- Angiotensin receptor blockers
- Maximal doses of amlodipine
Pharmacology
- Anti-ischemic effects
- Antianginal effects
- Inhibition of cardiac late sodium current (I Na)
- QT prolongation effect
- Inhibition of I Kr
- Absorption: C max 2-5 h
- Half-life: 6-22 h
- Distribution: 62% bound to human plasma proteins
- Metabolized by CYP3A and CYP2D6
- Metabolized in the liver and intestine
- Excretion in urine and feces
Dosage & Administration
- Initiate dosing at 500 mg twice daily
- Increase to 1000 mg twice daily if needed
- Take with or without meals
- Swallow tablets whole
Interaction
- CYP 3A Inhibitors
- CYP 3A Inducers
- P-gp Inhibitors (e.g., Cyclosporin)
- Drugs transported by P-gp or metabolized by CYP2D6
Contraindications
- Pre-existing QT prolongation
- Hepatic impairment
- Taking QT prolonging drugs
- Taking potent and moderately potent CYP3A inhibitors
Side Effects
- Cardiac Disorders: bradycardia, palpitations
- Ear and Labyrinth Disorders: tinnitus, vertigo
- Gastrointestinal Disorders: abdominal pain, dry mouth, vomiting
- Peripheral edema
- Respiratory, Thoracic, and Mediastinal Disorders: dyspnea
- Vascular Disorders: hypotension, orthostatic hypotension
Pregnancy & Lactation
- Pregnancy Category C
- No adequate studies assessing the effect on the fetus
- No well-controlled studies in pregnant women
- Excretion in human milk
- Potential risk to the fetus
- Potentiality for serious adverse reactions in nursing infants
Precautions & Warnings
- Blocks QTc and prolongs the QTc interval
- No increased risk of proarrhythmia or sudden death
- Co-administration with digoxin
- Increases the plasma concentrations of digoxin
- Caution when co-administering with P-gp inhibitors
Overdose Effects
- Dose-related increases in dizziness, nausea, and vomiting
- Diplopia
- Paresthesia
- Confusion
- Syncope
- Continuous ECG monitoring
- Hemodialysis is unlikely to be effective
