Indications
- Short-term management of moderate to severe acute post-operative pain
- Relieves pain, inflammation, and light sensitivity after ocular surgery
- Decreases eye inflammation or redness caused by seasonal allergies
Description
- Drug of the pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drug (NSAID)
- Chemical name: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2 - amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
- Inhibits synthesis of prostaglandins and acts as a peripherally acting analgesic
- Biological activity associated with the S-form
- Linear pharmacokinetic property
- Highly protein bound and largely metabolized in the liver
- Metabolites and some unchanged drugs are excreted in the urine
Pharmacology
- Potent analgesic non-steroidal anti-inflammatory drug (NSAID)
- Inhibits the cyclooxygenase enzyme system and prostaglandin synthesis
- Minimal anti-inflammatory effect at analgesic dose
Dosage & Administration
- Tablet recommended dose: 10 mg every 4-6 hours for short-term use (up to 7 days)
- Injection used for management of moderately severe, acute pain
- Bolus administration over no less than 15 seconds for IV administration
- IM administration should be given slowly and deeply into the muscle
- Analgesic effect begins within 30 minutes with maximum effect in 1 to 2 hours after dosing IV or IM
- Duration of analgesic effect is usually 4 to 6 hours
- Different dosing recommendations for adults, elderly, renally impaired patients, and pediatric patients
- Special administration instructions for eye drops
Interaction
- Increased side effects with other NSAIDs or aspirin
- Enhanced anti-coagulant effect with anti-coagulants
- Reduced anti-hypertensive effect with beta blockers
- Increased risk of renal impairment with ACE inhibitors
- Enhanced toxicity of methotrexate
Contraindications
- Hypersensitivity to ketorolac or other NSAIDs
- Use not recommended in children under 16 years of age
- Contraindicated as prophylactic analgesic before surgery

- Caution in patients with active or suspected peptic ulcer, gastrointestinal bleeding, asthma, and liver dysfunction
- Special precautions for patients using eye drops with corneal epithelial breakdown or wearing contact lenses
