Indications
- Treatment of reversible bronchospasm associated with chronic obstructive pulmonary disease (COPD)
- Treatment of reversible airway obstruction in acute and chronic asthma
Pharmacology
- Quaternary ammonium compound with anticholinergic properties
- Inhibition of vagal reflexes
- Antagonism of acetylcholine action
- Prevention of increase in intracellular concentration of cyclic GMP
Dosage
- Nebuliser Solution:
- Adults and children over 12 years of age: 250-500 micrograms 3 to 4 times daily
- For treatment of acute bronchospasm: 500 micrograms/2 ml
- Children 6-12 years of age: 250 micrograms up to a total daily dose of 1 mg
- Children 0-5 years of age: 125-250 micrograms up to a total daily dose of 1 mg
- Inhaler:
- Adults: 1-2 puffs three or four times daily
- Children 6-12 years: 1-2 puffs two to three times daily
- Children below 6 years: 1 puff three times daily
Administration
- Nebuliser Solution: Should be administered using a range of commercially available nebulizing devices. Solution should be used as soon as possible after mixing. Should be administered no more frequently than 6 hourly in children under 5 years of age.
- Inhaler: Correct and regular use is crucial to prevent or lessen the severity of asthma attacks. Cleaning instructions included.
Interaction
- Rinase has been used concomitantly with sympathomimetic bronchodilators, methylxanthines, steroids and cromolyn sodium without adverse drug reactions.
Contraindications
- Known hypersensitivity to atropine or its derivatives
- Known hypersensitivity to Ipratropium, Atropine or its derivative
- Contraindicated in patients with a history of hypersensitivity to soya lecithin or related food products such as soybean, lecithin and peanut

- Blurred vision/difficulty in accommodation
Pregnancy & Lactation
Safety during human pregnancy has not been established. Caution should be exercised when administered to nursing mothers.
Precautions & Warnings
- Nebuliser Solution: Use should be subject to close medical supervision during initial dosing. Caution advocated in patients with narrow-angle glaucoma, prostatic hyperplasia, bladder-outflow obstruction, or cystic fibrosis. Ocular complications may occur if solution/mist comes into contact with the eyes.
- Inhaler: Temporary blurring of vision, precipitation or worsening of narrow-angle glaucoma, or eye pain may result if aerosol is sprayed into the eyes. Immediate hypersensitivity reactions may occur.
Overdose Effects
Acute overdosage by inhalation is unlikely. Inhaled dosage of 5 mg produces an increase in heart rate and palpitation.
