Indications
- Long-term treatment of bronchospasm associated with chronic obstructive pulmonary disease (COPD)
- Treatment of chronic bronchitis
- Treatment of emphysema
Pharmacology
- Tiotropium is a long-acting antimuscarinic agent
- Exhibits pharmacological effects through inhibition of M3-receptors at the smooth muscle resulting in bronchodilation
- Prevention of methacholine-induced bronchoconstriction effects
Dosage & Administration
- Recommended dose for adults over 18 years: 18 mcg (1 capsule) once-daily
- Contents for oral inhalation only
- No dosage adjustment for geriatric, hepatically-impaired, or renally-impaired patients
- Close monitoring for anticholinergic effects in patients with moderate to severe renal impairment
Interaction
- Increased AUC 0-4 hour with cimetidine
- Decreased renal clearance with cimetidine
- No significant change with ranitidine
Contraindications
- History of hypersensitivity to atropine or its derivatives
- History of hypersensitivity to ipratropium
- History of hypersensitivity to any component of this product
Side Effects
- Most commonly reported adverse reaction: dry mouth
- Other possible anticholinergic effects: constipation, increased heart rate, blurred vision, glaucoma, urinary difficulty, urinary retention
Pregnancy & Lactation
- Pregnancy Category C
- Use during pregnancy only if potential benefit justifies potential risk to the fetus
- Safety and effectiveness not studied during labor and delivery
Precautions & Warnings
- May worsen symptoms and signs associated with narrow-angle glaucoma
- Should be used with caution in patients with prostatic hyperplasia or bladder-neck obstruction
Overdose Effects
- High doses may lead to anticholinergic signs and symptoms
- No systemic adverse effects following a single inhaled dose of up to 282 mcg in healthy volunteers
- Bilateral conjunctivitis and dry mouth seen following repeated once-daily inhalation of 141 mcg in healthy volunteers
