Indications

Pharmacology
- Dihydropyridine calcium-channel blocker
- Long duration of action
- Vasodilation
- Coronary blood flow increase
- Myocardial oxygen supply increase
Dosage & Administration
- Usual dose 5 mg once daily
- Maximum dose 10 mg once daily
- Administered without regard to meals
Interaction
- Potentially hazardous interactions with p-blockers in patients with impaired cardiac left ventricular function
- No interaction with Digoxin
- No interaction with Cimetidine
- No significant alteration of Warfarin effect on prothrombin time
- Food does not alter absorption rate
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnancy

- Peripheral edema
- Gastrointestinal disturbances
- Increased micturition frequency
- Lethargy
- Eye pain
- Mental depression
- Excessive fall in blood pressure
- Ischaemic chest pain

- Abnormalities in liver function
Pregnancy & Lactation
- Pregnancy Category C
- Use only if potential benefit justifies potential risk to fetus
- Discontinue nursing while administered
Precautions & Warnings
- Caution in patients with hepatic impairment, during pregnancy and breastfeeding
Use in Special Populations
- Lower starting dose for children with hypertension (6-17 years)
- Doses in excess of 5 mg not studied in pediatric patients
- Effect on blood pressure in children under 6 years not known
- Similar tolerability in elderly patients
- Normal dosage recommended for renal impairment
- Caution in dose selection for mild to moderate hepatic impairment
- Initiate at lowest dose for severe hepatic impairment
Overdose Effects
- Excessive peripheral vasodilatation
- Reflex tachycardia
- Systemic hypotension up to shock
- Active cardiovascular support required
- Elevation of extremities
- Monitoring of cardiac and respiratory function
- Attention to circulating fluid volume and urine output
- Vasoconstrictor may be helpful
- Intravenous calcium gluconate may be beneficial
- Gastric lavage may be worthwhile
- Use of charcoal within 2 hours to reduce absorption rate
- Dialysis unlikely to be of benefit
