Indications

Pharmacology
- Inhibition of microsomal protein synthesis
- Binding to the donor site on the 50S subunit of the bacterial ribosome
- Prevention of translocation to the donor site
- Activity against most Gram-positive bacteria
- Activity against Chlamydia and some Gram-negative bacteria
- Better acid-stability compared to Erythromycin
- Twice the activity against H. influenzae compared to Erythromycin

- Avoidance in patients with known QT prolongation or receiving drugs known to prolong the QT interval
- Discontinuation upon signs and symptoms of hepatitis
- Increased risk of all-cause mortality in patients with coronary artery disease
Use in Special Populations
- Safety and effectiveness in pediatric patients 6 months and older
- Prevention of disseminated Mycobacterium avium complex (MAC) disease in pediatric patients 20 months and older with advanced HIV infection
- Lack of study of safety in MAC patients under 20 months
- No established safety and effectiveness in pediatric patients under 6 months
Overdose Effects
- Expected gastrointestinal symptoms with large amounts
- Case study of altered mental status, paranoid behavior, hypokalemia, and hypoxemia with 8 grams ingestion
- Adverse reactions treatment with prompt drug elimination and supportive measures
- Insensitivity of Clarithromycin serum levels to haemodialysis or peritoneal dialysis
