Indications

Pharmacology

  • Inhibition of microsomal protein synthesis
  • Binding to the donor site on the 50S subunit of the bacterial ribosome
  • Prevention of translocation to the donor site
  • Activity against most Gram-positive bacteria
  • Activity against Chlamydia and some Gram-negative bacteria
  • Better acid-stability compared to Erythromycin

  • Avoidance in patients with known QT prolongation or receiving drugs known to prolong the QT interval
  • Discontinuation upon signs and symptoms of hepatitis
  • Increased risk of all-cause mortality in patients with coronary artery disease

Use in Special Populations

  • Safety and effectiveness in pediatric patients 6 months and older
  • Prevention of disseminated Mycobacterium avium complex (MAC) disease in pediatric patients 20 months and older with advanced HIV infection
  • Lack of study of safety in MAC patients under 20 months
  • No established safety and effectiveness in pediatric patients under 6 months

Overdose Effects

  • Expected gastrointestinal symptoms with large amounts
  • Case study of altered mental status, paranoid behavior, hypokalemia, and hypoxemia with 8 grams ingestion
  • Adverse reactions treatment with prompt drug elimination and supportive measures
  • Insensitivity of Clarithromycin serum levels to haemodialysis or peritoneal dialysis
