Indications
- Symptomatic relief of Osteoarthritis (OA)
- Symptomatic relief of Rheumatoid arthritis (RA)
- Symptomatic relief of Ankylosing spondylitis
- Symptomatic relief of pain and signs of inflammation associated with acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Inhibition of COX-2 decreases clinical signs and symptoms with decreased potential for gastrointestinal (GI) toxicity and effects on platelet aggregation
- Selective inhibition of COX-2 decreases pain, inflammation, and fever
- Dose-dependent inhibition of COX-2 without inhibition of COX-1
Dosage & Administration
- Recommended dose for Osteoarthritis: 30 mg once daily, can be increased to 60 mg once daily in some patients
- Recommended dose for Rheumatoid arthritis: 90 mg once daily
- Recommended dose for Ankylosing spondylitis: 90 mg once daily
- Recommended dose for Acute gouty arthritis: 120 mg once daily, for 8 days in clinical trials
Interaction with medicine
- Increase in prothrombin time when used with oral anticoagulants
- Potential reduction in the effect of diuretics and other antihypertensive drugs when used with NSAIDs
- Can be used concomitantly with low-dose acetylsalicylic acid for cardiovascular prophylaxis
Contraindications
- Hypersensitivity to the active substance or to any of the excipients
- Active peptic ulceration or active gastrointestinal (GI) bleeding
- History of allergic-type reactions after taking acetylsalicylic acid or NSAIDs including COX-2 inhibitors
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years of age
- Inflammatory bowel disease
- Congestive heart failure (NYHA ll-IV)
- Uncontrolled hypertension
- Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Not recommended for use by breastfeeding mothers
Precautions & Warnings
- Caution with patients at risk of developing gastrointestinal complications with NSAIDs
- Consideration for patients with significant risk factors for cardiovascular events
- Monitoring of renal function in patients with reduced prostaglandin formation
- Caution in patients with history of cardiac failure, left ventricular dysfunction, or hypertension
- Monitoring of liver function and discontinuation if signs of hepatic insufficiency occur
- Discontinuation at the first appearance of hypersensitivity signs
- Caution when co-administering with warfarin or other oral anticoagulants
Overdose Effects
- No significant toxicity observed with single doses up to 500 mg and multiple doses up to 150 mg/day for 21 days
- Usual supportive measures recommended in case of overdose
