Indications

Pharmacology
Selective and competitive inhibitor of HMG-CoA reductase, enhances LDL receptor uptake and catabolism, inhibits hepatic synthesis of VLDL and LDL particles
Dosage administration
- Usual dose range of 5-20 mg/day
- Maximum recommended dose is 20 mg/day
- 40 mg once daily for very high cholesterol and high risk of MI or stroke
- 20 mg once daily for homozygous familial hypercholesterolemia
- 5 mg once daily for Asian patients
- Contraindicated with certain lipid-lowering therapies
- Dosage adjustments for severe renal impairment
Interaction
- Enhanced exposure with cyclosporine
- Combination with gemfibrosil should be avoided
- Enhanced exposure with lopinavir/ritonavir or atazanavir/ritonavir
- Prolongs international normalized ratio with coumarin anticoagulants
- Enhanced risk of skeletal muscle effects with concomitant lipid-lowering therapies

- Known hypersensitivity to product components
- Liver disease

Pregnancy lactation
Administered to women of childbearing age only when conception is highly unlikely and potential hazards are understood; Discontinue if pregnancy occurs; Unknown excretion in human milk
Precautions warnings
- Prescribed with caution in patients with predisposing factors for myopathy
- Discontinue if markedly elevated creatinine kinase levels or myopathy occurs
- Temporarily withheld in patients with acute, serious condition suggestive of myopathy
- Liver enzyme tests recommended before and following initiation of therapy
