Emaglin-L 25 mg+5 mg (Tablet)

Indications

• Adjunct to diet
• Improve glycemic control
• Type 2 diabetes mellitus

Pharmacology

• Inhibits DPP-4 enzyme
• Increases insulin biosynthesis
• Reduces glucagon secretion
• Reduces renal reabsorption of glucose

Dosage & Administration

• Recommended dose: 10 mg Empagliflozin and 5 mg Linagliptin once daily, morning
• Increased dose: 25 mg Empagliflozin and 5 mg Linagliptin once daily
• Assess renal function before initiating
• Do not initiate if eGFR below 45 mL/min/1.73 m²
• Discontinue if eGFR falls below 45 ml/min/1.73 m²

Interaction

• Diuretics: Increased urine volume and frequency
• Insulin or Insulin Secretagogues: Increased risk for hypoglycemia
• Inducers of P-glycoprotein or CYP3A4 Enzymes: Decreased exposure
• Positive Urine Glucose Test: Increased urinary glucose excretion
• Interference with 1,5-anhydroglucitol (1,5-AG) Assay: Unreliable measurements

Contraindications

• Severe renal impairment
• Hypersensitivity reaction to Linagliptin or Empagliflozin

Pregnancy & Lactation

• Not recommended during second and third trimesters
• Limited data on pregnant women
• Risks to mother and fetus with poorly controlled diabetes

Precautions & Warnings

• Pancreatitis: Acute and fatal cases reported
• Ketoacidosis: Serious condition requiring hospitalization
• Volume Depletion: May manifest as hypotension or changes in creatinine
• Urosepsis and Pyelonephritis: Reports of serious infections
• Hypoglycemia with Insulin: Known to cause hypoglycemia
• Necrotizing Fasciitis: Life-threatening infection
• Genital Mycotic Infections: Increased risk
• Heart Failure: Association observed in trials

• Usual supportive measures
• Removal by hemodialysis not studied
