Indications
- Hypertension
- Angina

Pharmacology
- Selective ß1 blocker
- Reduces heart rate and cardiac output
- Decreases arterial hypertension
- Affects lipid metabolism
- Extremely low inter and intra-individual variability of plasma concentration profiles
- Metabolized via oxidative pathways
- Primarily metabolized via CYPSA4
- Renally eliminated
- Elimination half-life of 10-12 hours
Dosage & Administration
- Usual starting dose of 5 mg once daily
- Increased to 10 mg if 5 mg is inadequate
- Maximum dose of 20 mg once daily
- Titration interval of 2 weeks
- Instructions for patients with renal or hepatic impairment
- No adjustment usually necessary for elderly
Interaction
- Not recommended with other β-blocking agents
- Monitoring required when used with catecholamine-depleting drugs
- Care needed when used with centrally active antihypertensive agents
- Usage precautions with antiarrhythmic agents and calcium channel blockers
Contraindications
- Cardiogenic shock
- Overt heart failure
- Second or third degree A-V block
- Right ventricular failure secondary to pulmonary hypertension
- Sinus bradycardia

- Use caution in patients with impaired renal or hepatic function
- Increased reactivity to allergens in patients with a history of severe anaphylactic reaction
