Pharmacology

Rabeprazole suppresses gastric acid secretion by inhibiting the gastric H+/K+-ATPase at the secretory surface of the gastric parietal cell.

Dosage

  • Recommended oral doses for active duodenal ulcer and active benign gastric ulcer
  • Recommended oral dose for erosive or ulcerative gastro-esophageal reflux disease (GERD)
  • Recommended oral dose for GERD long-term management (GERD maintenance)
  • Recommended oral dose for treatment of moderate to very severe GERD
  • Recommended oral dose for Zollinger-Ellison Syndrome
  • Dosing recommendation for treatment of H. pylori in peptic ulcer disease

Administration

Rabeprazole tablets should be taken in the morning, before eating, swallowed whole, not chewed or crushed.

Interaction

  • Potential interaction with pH-dependent compounds
  • Co-administration effects with ketoconazole or itraconazole
  • No interaction observed with liquid antacids
  • Absorption-based interaction with atazanavir

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Contraindicated in pregnancy and breastfeeding

  • Possible increase in risk of hip, wrist, and spine fracture
  • Risk of cross-hypersensitivity reactions
  • Hepatic enzyme abnormalities observed
  • Caution advised in patients with severe hepatic dysfunction
  • Co-administration with atazanavir not recommended
  • Possible increase in risk of gastrointestinal infections
  • Severe hypomagnesaemia reported in patients on prolonged treatment
  • Potential reduction in vitamin B12 absorption
  • Association with subacute cutaneous lupus erythematosus (SCLE)
  • Interference with laboratory tests

Use in Special Populations

  • No dosage adjustment necessary for renal or hepatic impairment
  • Not recommended for use in children

Overdose Effects

  • Effects generally minimal and reversible without medical intervention
