Indications

  • Treatment of elevated intraocular pressure
  • Patients with ocular hypertension
  • Open-angle glaucoma

Pharmacology

  • Non-selective β-adrenergic receptor blocker
  • Reduction of aqueous humour formation
  • Lowering of blood pressure

Dosage & Administration

  • Initial dose: 1 drop of 0.25% solution bid
  • May increase to 1 drop of 0.5% solution bid if inadequate response
  • Decrease to 1 drop once daily if controlled
  • Do not exceed 1 drop bid of 0.5% solution

Interaction

  • Mydriasis with concomitant therapy with epinephrine
  • Interactions with beta-adrenergic blocking agents
  • Calcium antagonists
  • Catecholamine-depleting drugs
  • Digitalis
  • Quinidine
  • Clonidine
  • Injectable epinephrine

Contraindications

  • Bronchial asthma
  • Severe chronic obstructive pulmonary disease
  • Sinus bradycardia
  • Second or third degree atrioventricular block
  • Overt cardiac failure
  • Cardiogenic shock
  • Hypersensitivity to any component

  • Burning and stinging sensation of the eyes
  • Bradycardia
  • Hypotension
  • Arrhythmia and AV or SA nodal block
  • CHF
  • Pulmonary oedema
  • Raynaud's phenomenon
  • Headache
  • Dizziness
  • Fatigue
  • Asthenia
  • Abdominal discomfort
  • Nausea
  • Constipation
  • Hypoglycaemia

Pregnancy & Lactation

  • Use during pregnancy if potential benefit justifies potential risk to the fetus
  • Detection in breast milk
  • Serious adverse reactions in nursing infants
  • Consideration whether to discontinue nursing or drug

Precautions & Warnings

  • Inadequate cardiac function
  • Diabetes mellitus
  • Myasthenia gravis
  • Cerebrovascular insufficiency
  • History of atopy
  • Avoid abrupt withdrawal
  • Exacerbation of angina symptoms
  • Precipitation of MI in patients with coronary artery disease
  • Precipitation of thyroid crisis in patients with thyrotoxicosis
  • Patients undergoing major surgery
  • Masking signs of hyperthyroidism and hypoglycemia
  • Not for monotherapy for angle-closure glaucoma
  • Renal and hepatic impairment

Overdose Effects

  • Systemic effects similar to those seen with systemic beta-adrenergic blocking agents
  • Dizziness
  • Headache
  • Shortness of breath
  • Bradycardia
  • Bronchospasm
  • Cardiac arrest

Therapeutic Class

  • Drugs for miotics and glaucoma
