Indications
- Treatment of adults with type 2 diabetes mellitus
- Adjunct to diet and exercise
- Patients insufficiently controlled on Metformin alone
- Combination with other medicinal products for diabetes
- Patients already being treated with Empagliflozin and Metformin separately
Pharmacology
- Empagliflozin is an inhibitor of Sodium-Glucose Co-Transporter 2 (SGLT2)
- Metformin Hydrochloride is a biguanide type oral antihyperglycemic drug
- Reduces renal reabsorption of glucose
- Lowers the renal threshold for glucose
- Increases urinary glucose excretion
- Lowers basal and postprandial plasma glucose
- Does not produce hypoglycemia
Dosage & Administration
- Dosage individualized based on effectiveness and tolerability
- Take twice daily with meals
- Dose escalation should be gradual
- Maximum recommended daily dose of Metformin Hydrochloride is 2000 mg
- Maximum recommended daily dose of Empagliflozin is 25 mg
- Extended-release formulations available
- Adjust dosing based on effectiveness and tolerability
- Renal impaired patient assessment
Interaction
- Diuretics co-administration leads to increased urine volume
- Insulin or Insulin Secretagogues co-administration increases risk for hypoglycemia
- Monitoring glycemic control with urine glucose tests not recommended
- Drugs that Reduce Metformin Clearance may increase the accumulation of Metformin
- Carbonic Anhydrase Inhibitors may increase risk of lactic acidosis
- Alcohol can potentiate the effect of Metformin on lactate metabolism

- Hypersensitivity to Empagliflozin and Metformin
- Acute metabolic acidosis
- Diabetic pre-coma
- Severe renal failure
- Acute conditions altering renal function
- Diseases causing tissue hypoxia
- Hepatic impairment
- Acute alcohol intoxication
