Indications

  • Reduction of elevated total cholesterol
  • Reduction of LDL cholesterol
  • Reduction of triglycerides
  • Increase in HDL cholesterol
  • Treatment of primary hyperlipidemia
  • Treatment of homozygous familial hypercholesterolemia

Pharmacology

  • Atorvastatin lowers plasma cholesterol
  • Ezetimibe inhibits cholesterol absorption by the small intestine
  • Atorvastatin reduces LDL production
  • Ezetimibe reduces blood cholesterol levels

Dosage & Administration

  • Dosage range: 10/10 mg to 80/20 mg daily
  • Recommended starting dose: 10/10 mg or 20/10 mg daily
  • Recommended starting dose for patients needing larger LDL reduction: 40/10 mg daily
  • Dosage for patients with homozygous familial hypercholesterolemia: 40/10 mg or 80/10 mg daily
  • No dosage adjustment necessary for patients with renal impairment

Interaction

  • Risk of myopathy with concurrent fibric acid derivatives
  • Avoid coadministration with cyclosporine
  • Risk of myopathy enhanced with niacin combination
  • Monitor patients taking digoxin
  • Consider AUC values when selecting an oral contraceptive
  • Exercise caution when prescribing with colchicine
  • Appropriate monitoring when added to warfarin

Contraindications

  • Active liver disease
  • Hypersensitivity to any component

  • Rhabdomyolysis
  • Myopathy
  • Liver enzyme abnormalities
  • Myalgia
  • Abdominal pain

Pregnancy & Lactation

  • Pregnancy Category X
  • Contraindicated in pregnant women
  • No benefit during pregnancy
  • Not known if excreted in human milk

Precautions & Warnings

  • Cases of myopathy reported with colchicine coadministration
  • Caution with concurrent drugs affecting steroid hormones
  • Monitor liver enzymes prior to therapy
  • Caution with drugs decreasing endogenous steroid hormones

Use in Special Populations

  • Safety and effectiveness not established in pediatric patients
  • No dosage adjustment needed in geriatric patients
