Indications

• First line therapy for advanced ovarian carcinoma
• Treatment of breast carcinoma
• Treatment of non-small cell lung carcinoma
• Second line treatment of AIDS-related Kaposi's Sarcoma
• Treatment of Gastric Carcinoma

Pharmacology

• Promotes microtubule assembly
• Stabilizes microtubules
• Inhibition of dynamic reorganization of microtubule network
• Induces abnormal arrays of microtubules
• Declines in plasma concentrations in a biphasic manner

Dosage & Administration

• Premedication required before administration
• Different dosages based on cancer type and line of treatment
• 24-hour and 3-hour intravenous infusions
• Dose adjustments based on patient tolerance
• Precautions for neutrophil and platelet counts

Interaction

• No affected clearance by cimetidine premedication
• Effects of co-administration with cisplatin and doxorubicin
• Metabolism catalyzed by cytochrome P450 isoenzymes
• Caution with concomitant use of specific medicines

Contraindications

• Severe hypersensitivity reactions
• Lactation
• Baseline neutrophil and platelet levels
• Severe hepatic impairment

Side Effects

• Low blood counts
• Hair loss
• Peripheral neuropathy
• Nausea and vomiting
• Hypersensitivity reactions

Pregnancy & Lactation

• Pregnancy Category D
• Contraindicated during lactation

• Administered under the supervision of an experienced physician
• Possible extravasation
• Significant hypersensitivity reactions
• Bone marrow suppression monitoring
• Severe cardiac conduction abnormalities

Use in Special Populations

• Dose reduction for severe neutropenia
• Caution in hepatic impairment
• Not recommended for pediatric use

Overdose Effects

• No known antidote
• Treatment directed at primary anticipated toxicities
• Closely monitor patient

Reconstitution

• Must be diluted prior to infusion
• Stable for up to 27 hours at ambient temperature
• Visual inspection of solution prior to administration
