Indications
- Treatment of hormone receptor (HR)-positive, human epidermal growth factor receptor 2 (HER2)-negative advanced or metastatic breast cancer
- Use in combination with an aromatase inhibitor as initial endocrine-based therapy in postmenopausal women
- Use in combination with fulvestrant in women with disease progression following endocrine therapy
Pharmacology
- Inhibitor of Cyclin-dependent kinases (CDK) 4 and 6
- Reduces cellular proliferation of Estrogen receptor (ER)-positive breast cancer cell lines
- Blocks progression of the cell from G1 into s phase of the cell cycle
- Leads to decreased retinoblastoma (RB) protein phosphorylation
- Results in increased cell senescence
Dosage & Administration
- Recommended dose of 125 mg capsule taken orally once daily for 21 consecutive days followed by 7 days off
- Should be taken with food
- Treatment with an LHRH agonist for men treated with combination Palbociclib plus aromatase inhibitor therapy
Interaction
- Primarily metabolized by CYP3A and Sulfotransferase (SULT) enzyme SULT2A1
- Time-dependent inhibitor of CYP3A
- Effects on plasma concentrations with CYP3A inhibitors and inducers
- Altered plasma concentrations of sensitive CYP3A substrate drugs

- Nausea
- Hair loss
- Diarrhea
Pregnancy & Lactation
- Can cause fetal harm when administered to a pregnant woman
- Risk of major birth defects and miscarriage in clinically recognized pregnancies
Precautions & Warnings
- Neutropenia as the most frequently reported adverse reaction
- Incidence of neutrophil counts and grade 3 neutropenia
- Interstitial Lung Disease (ILD)/Pneumonitis
- Embryo Fetal Toxicity
Overdose Effects
- No known antidote for Palbociclib
- Treatment consists of general supportive measures
