Indications
- Neutralising the immediate toxic effects of folic acid antagonists, e.g. methotrexate

- 6.249 mg Calcium Folinate Hydrate BP equivalent to 5 mg Folinic Acid

Pharmacology
- Folinic acid is a metabolite and active form of folic acid involved in 1-carbon transfer reactions
- Potent antidote for hematopoietic and reticuloendothelial toxic effects of folic acid antagonists
- Principle of high-dose methotrexate therapy with 'Folinate rescue'
Dosage & Administration
- Varied dosages for calcium folinate rescue treatment depending on methotrexate dose
- Administration of 5 mg to 15 mg of Folinic Acid per day for megaloblastic anaemia
- No established safety and efficacy of folinic acid in children and adolescents
Interaction
- Potential counteraction of antiepileptic effect of phenobarbitone, phenytoin, and primidone by large amounts of folinates
- Caution required with concurrent administration of calcium folinate with fluoropyrimidine
Contraindications
- Hypersensitivity to calcium folinate or any other components
- Pernicious anemia or other megaloblastic anemia with deficient vitamin B12
- Rare hereditary problems of galactose intolerance, lapp lactase deficiency, or glucose-galactose malabsorption

Pregnancy & Lactation
- No adequate and well-controlled clinical studies in pregnant women
- Caution required during administration to nursing mothers
Precautions & Warnings
- Use only with methotrexate or 5-FU under supervision of experienced clinician
- Prompt administration required for inadvertent overdosage of a folic acid antagonist
- Do not give simultaneously with folic acid antagonists to abort clinical toxicity
Overdose Effects
- Nullify the chemotherapeutic effect of folic acid antagonists
- No specific antidote to calcium folinate overdose
- Supportive care required for overdosage
