Indications

  • Treatment of major depressive disorder (MDD)

Pharmacology

  • Enhances central noradrenergic and serotonergic activity
  • Acts as an antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
  • Potent antagonist of 5-HT<sub>2</sub> and 5-HT<sub>3</sub> receptors
  • Antagonist of histamine (H<sub>1</sub>) receptors
  • Moderate peripheral &alpha;1-adrenergic antagonist
  • Moderate antagonist at muscarinic receptors
  • Rapid and well-absorbed after oral administration
  • Plasma protein binding of approximately 85%
  • Half-life of elimination is 20-40 hours
  • Display linear pharmacokinetics within recommended dose range
  • Extensively metabolized and eliminated via urine and faeces

Dosage & Administration

  • Recommended starting dose of 15 mg/day
  • Effective dose range of 15 to 45 mg/day
  • Patients not responding to 15 mg dose may benefit from dose increases up to 30-45 mg/day
  • Elimination half-life of approximately 20 to 40 hours
  • Dose changes to be made at intervals of less than 1 to 2 weeks
  • Not recommended for use in children
  • Instructions for missed dose

Interaction

  • Clinically significant drug-drug interactions with MAO inhibitors and other serotonergic drugs
  • May interrupt the metabolism or activity of Carbamazepine, Phenytoin, or Cimetidine
  • Avoidance of Alcohol &amp; Diazepam

Contraindications

  • Contraindicated in patients with known hypersensitivity to Mirtazapine or its excipients
  • Concomitant use with MAO inhibitors
