Indications

Pharmacology
- Belongs to dibenzothiazepine derivatives
- Antagonist at multiple neurotransmitter receptors
- Antagonist at serotonin 5HT1A and 5HT2, dopamine D1 and D2, histamine H1, and adrenergic α1 and α2 receptors
Dosage & Administration
- Administered twice daily
- First four days dose titration
- Usual effective dose range of 300-450 mg/day
- Titration to the usual effective dose range of 300-450 mg/day
- Usual effective dose range of 400-800 mg/day
- Dosage adjustments up to 800mg/day
Interaction
- Caution with medicines causing electrolyte imbalance
- Caution with medicines increasing QT interval
- Co-administration with thioridazine or carbamazepine
- Co-administration with phenytoin
Contraindications
- Contra-indicated in patients hypersensitive to it

- Somnolence
- Dizziness
- Dry mouth
- Withdrawal symptoms
- Elevations in serum triglyceride levels
- Elevations in total cholesterol
- Decreases in HDL cholesterol
- Weight gain
- Decreased haemoglobin
- Extrapyramidal symptoms
Pregnancy & Lactation
- Safety and efficacy not established during human pregnancy
- Use during pregnancy only if benefits justify potential risks
- Avoid breastfeeding while taking
Precautions & Warnings
- Increased risk of suicidal thoughts and self-harm
- Caution in patients with known cardiovascular disease
- Caution with patients with a history of seizures
- Consider dose reduction or discontinuation in case of tardive dyskinesia
- Discontinue if neuroleptic malignant syndrome appears
- Exercise caution in patients with cardiovascular disease or family history of QT prolongation
- Risk of severe neutropenia
- Risk of hyperglycemia & diabetes mellitus
Use in Special Populations
- Caution in the elderly
- Not indicated for use in children and adolescents below 18 years of age
Overdose Effects
- Survival reported in acute overdoses of up to 30 grams
- No specific antidote
- Intensive care procedures recommended
- Close medical supervision and monitoring
