Indications

Pharmacology
Tadalafil is a selective phosphodiesterase type 5 (PDE5) inhibitor. Inhibition of PDE5 increases cGMP in smooth muscle cells, causing smooth muscle relaxation and increased blood flow into the corpus cavernosum, leading to penile erection. It also relaxes smooth muscles in the prostate and bladder, improving blood flow to the urinary tract and widening the opening of the bladder neck, resulting in improved voiding.
Dosage & Administration
- Erectile Dysfunction: 10 mg starting dose, can be increased to 20 mg or decreased to 5 mg, maximum dosing frequency once daily, effective up to 36 hours
- Benign prostatic hyperplasia: 5 mg taken at the same time every day
- Combined Erectile Dysfunction and Benign prostatic hyperplasia: 5 mg at the same time every day
Interaction
May interact with Nitrates, Alpha adrenergic blockers, Antihypertensives, Alcohol, Antacids, Ketoconazole, Ritonavir, Erythromycin, Itraconazole, Grapefruit juice, Rifampin, Carbamazepine, Phenytoin, Phenobarbital

Pregnancy & Lactation
Tadalafil is assigned to pregnancy category B by the USFDA. Use during pregnancy is only recommended when benefit outweighs risk. Caution should be used when administering tadalafil to nursing women.
