Indications

  • Treatment of chronic iron overload due to repeated blood transfusions
  • Indicated for thalassemia, sickle cell disease, myelodysplastic syndrome
  • Treatment of chronic iron overload in non-transfusion-dependent thalassemia syndromes

Pharmacology

  • Orally active chelator
  • Highly selective for iron
  • Binds iron in a 2:1 ratio
  • Promotes excretion of iron in faeces
  • Low affinity for zinc and copper
  • Mean net excretion at different daily doses

Dosage

  • Initial daily dose of 20-40 mg/kg body weight
  • Administration on an empty stomach
  • Dispersion in water or juice
  • Monitoring of serum ferritin
  • Dose adjustments based on trends in serum ferritin

Administration

  • Taken once daily on an empty stomach
  • Dispersed in water or juice
  • No chewing of tablets
  • Avoiding carbonated drinks or milk for dispersion

Interaction

  • Avoidance with aluminium-containing antacids
  • Caution with drugs metabolized through CYP3A4
  • Potential dose increase with certain concomitant drugs
  • Caution with drugs with ulcerogenic potential or anticoagulants

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Creatinine clearance less than 40 ml/min
