Description

  • A synthetic, antibacterial agent with in vitro activity against Gram positive aerobic bacteria, some Gram positive anaerobic bacteria, and certain Gram negative bacteria.
  • Selectively inhibits bacterial protein synthesis via a mechanism of action different from that of other antibacterial agents.
  • Available in parenteral formulation for intravenous administration and oral presentation in the form of film-coated tablets and oral suspension.

Indications

  • Vancomycin-Resistant Enterococcus faecium infections
  • Nosocomial pneumonia caused by Staphylococcus aureus and Streptococcus pneumoniae
  • Complicated skin and skin structure infections including diabetic foot infections
  • Community-acquired pneumonia
  • Bacteremia

Pharmacology

  • Binds to the 23S ribosomal RNA of the 50S subunit of the bacterial ribosome
  • Prevents the formation of a functional 70S initiation complex
  • Bacteriostatic against enterococci and staphylococci
  • Bactericidal for the majority of streptococci strains

Dosage

  • Oral bioavailability of approximately 100%
  • Injection administered over a period of 30 to 120 minutes
  • Dosage varies for pediatric patients, adults, adolescents, and neonates

Contraindications

  • Known hypersensitivity to Linezolid or any of the product components
  • Concurrent use of monoamine oxidase inhibitors
  • Uncontrolled hypertension, pheochromocytoma, thyrotoxicosis, carcinoid syndrome
  • Use with sympathomimetic, vasopressive, dopaminergic agents, serotonin re-uptake inhibitors, tricyclic antidepressants, and other specific medications

Side Effects

  • Mild to moderate adverse events
  • Common events include diarrhea, headache, and nausea
  • Other events such as oral and vaginal moniliasis, hypertension, dyspepsia, localized abdominal pain, pruritus, and tongue discoloration

Pregnancy & Lactation

  • Pregnancy Category C
  • Caution advised during lactation due to potential excretion in human milk

Precautions & Warnings

  • Monitoring for recurrent nausea or vomiting, unexplained acidosis, and low bicarbonate level
  • Observation for signs and symptoms of serotonin syndrome
  • Prompt ophthalmic evaluation if visual impairment symptoms occur
  • Risk of convulsions in some cases with a history of seizures or risk factors

Overdose Effects

  • No reported cases of overdose
  • Advised symptomatic and supportive care
  • Approximately 30% removal during 3 hours of haemodialysis
