Indications
- Replacement therapy in hypothyroidism

Pharmacology
- Contains synthetic Levothyroxine
- Identical to the natural hormone T4
- Conversion of T4 to T3 in peripheral tissues
- Major carrier of T4 is Thyroxine Binding Globulin
- Long half-life in circulation
- Liver as the major site of degradation of Thyroid hormones
- Enterohepatic circulation of the Thyroid hormones
Dosage & Administration
- Initial starting dose: 25-50 mcg/day
- Gradual increments in dose at 6-8 week intervals
- Different dosages for severe hypothyroidism
- Titration of dose for pituitary or hypothalamic hypothyroidism
- Recommended starting dose for newborns and children
- Dose adjustment based on clinical response and laboratory parameters
Interaction
- Concurrent use with tri/tetracyclic antidepressants
- Addition of T4 to antidiabetic or insulin therapy
- Careful monitoring of diabetic control recommended
- Reduced serum digitalis glycoside levels in hyperthyroidism
Contraindications
- Untreated subclinical or overt Thyrotoxicosis
- Acute Myocardial Infarction
- Uncorrected Adrenal failure
Side Effects
- Adverse reactions primarily those of hyperthyroidism
- General, central nervous system, musculoskeletal, cardiovascular, respiratory, gastrointestinal, and dermatologic reactions
Pregnancy & Lactation
- Pregnancy Category A
- Pregnancy may increase Levothyroxine requirements
- Caution in administration to nursing women
- Adequate replacement doses generally needed to maintain normal lactation
Precautions & Warnings
- Adrenocortical insufficiency in pituitary hypothyroidism
- Careful dosing in cardiovascular disease
- Adjustment of dosage with marked change in body weight
- Monitoring of blood levels of T3 and T4
Overdose Effects
- Signs and symptoms of hyperthyroidism
- Agitation, confusion, irritability, hyperactivity, headache, sweating, tachycardia, arrhythmias, tachypnoea, pyrexia, increased bowel movements, convulsions
- Cerebral embolism, shock, coma, and death reported
