Indications

  • Control of acute asthma
  • Management of chronic asthma (for both symptomatic and prophylactic treatment)
  • Controlling nocturnal asthma and early morning wheezing
  • Management of chronic obstructive lung disease (chronic bronchitis and emphysema)
  • Control of apnea of pre-maturity

Pharmacology

  • Bronchodilator
  • Methylxanthine
  • Smooth muscle relaxation
  • Suppression of airways response to stimuli
  • Increases force of contraction of diaphragmatic muscles
  • Half-life ranges from 3 to 9 hours in adult nonsmokers with uncomplicated asthma

Dosage and administration

  • Individualized dose based on peak serum theophylline concentration measurements
  • Sustained release preparations may be administered every 12 hours in adults
  • Recommended maintenance dose based on age and weight

Interaction

  • Not to be used concurrently with other preparations containing xanthine derivatives
  • Clearance of Contine is increased by certain medications
  • Clearance of the drug is reduced by certain medications
  • Can potentiate hypokalemia resulting from beta-2-agonist therapy, steroids, diuretics, and hypoxia

Contraindications

  • Hypersensitivity to Theophylline or any other component of the product

Side effects

  • Gastrointestinal discomfort
  • Headache
  • Nausea
  • Insomnia
  • Hypotension
  • CNS stimulation (especially in children)
  • Diuresis (especially in children)

Pregnancy and lactation

  • Potential risk to pregnant women
  • Excreted into breast milk
  • May cause irritability or signs of mild toxicity in nursing human infants
  • Serious adverse effects in the infant are unlikely unless the mother has toxic serum Theophylline concentrations

Precautions and warnings

  • Careful consideration needed for various interacting drugs and physiologic conditions
  • Dosage adjustment required prior to initiation of Contine therapy and during follow up
  • Low initial dose recommended, increased slowly if tolerated

Use in special populations

  • Low clearance in neonates
  • Careful attention to dosage selection and monitoring of serum Contine concentrations in pediatric patients
