Indications

  • Treatment of chronic angina
  • May be used with beta-blockers, nitrates, calcium channel blockers, anti-platelet therapy, lipid-lowering therapy, ACE inhibitors, and angiotensin receptor blockers
  • Decreases angina episodes in patients with coronary artery disease
  • Effects on angina rate or exercise tolerance may vary in women and men

Pharmacology

  • Anti-ischemic effects
  • Anti-anginal effects
  • Inhibition of cardiac late sodium current (I_Na)
  • Prolongs the QT interval
  • Inhibition of I_Kr
  • Absorption: C_max - 2-5 h
  • Half-life: 6-22 h
  • Approximately 62% bound to human plasma proteins
  • Metabolized mainly by CYP3A and to a lesser extent by CYP2D6
  • Metabolized rapidly and extensively in the liver and intestine
  • Excreted in urine and feces

Dosage & Administration

  • Initial dose of 500 mg twice daily
  • Maximum recommended daily dose of 1000 mg twice daily
  • Swallow whole, do not crush, break or chew
  • Pediatric use not established

Interaction

  • Avoid use with strong CYP 3A inhibitors
  • Limit maximum dose with moderate CYP 3A inhibitors
  • Adjust dosage with P-gp Inhibitors (e.g., Cyclosporin)
  • May require reduced doses of drugs transported by P-gp or metabolized by CYP2D6 when used together

Contraindications

  • Pre-existing QT prolongation
  • Hepatic impairment
  • Taking QT prolonging drugs
  • Taking potent and moderately potent CYP3A inhibitors

  • Abdominal pain
  • Dry mouth
  • Vomiting
  • Peripheral edema

Pregnancy & Lactation

  • Category C - potential risk to the fetus
  • Use during pregnancy when benefit justifies risk
  • Not known whether excreted in human milk
  • Consider discontinuing nursing or drug, taking into account importance to the mother

Precautions & Warnings

  • Blocks QTc and prolongs the QTc interval
  • Experience in acute coronary syndrome population did not show increased risk of proarrhythmia or sudden death
  • Increased plasma concentrations of digoxin when co-administered
  • Caution with co-administration with P-gp inhibitors like ritonavir or cyclosporine

Overdose Effects

  • Dose-related increases in dizziness, nausea, and vomiting
  • Produces diplopia, paresthesia, confusion, and syncope with high intravenous exposure
  • Continuous ECG monitoring may be warranted
  • Unlikely to be cleared by hemodialysis
