Indications
- Prophylaxis and chronic treatment of asthma
- Acute prevention of Exercise-Induced Bronchoconstriction (EIB)
- Relief of symptoms of Allergic Rhinitis (AR): Seasonal & Perennial Allergic Rhinitis
Pharmacology
- Montelukast is a selective and orally active leukotriene receptor antagonist
- Inhibits the cysteinyl leukotriene receptor (CysLT1)
- Correlated with the pathophysiology of asthma & allergic rhinitis
Dosage & Administration
- Adult dosage: 10 mg tablet once daily
- Pediatric dosage: 4-5 mg tablet once daily based on age
- No dosage adjustment needed for hepatic or renal insufficiency
- Similar pharmacokinetic profile in elderly and younger adults
Interactions
- Administered with other asthma therapies without increase in adverse reactions
- No clinically important effects observed with multiple drugs
- Appropriate clinical monitoring when co-administered with potent cytochrome P450 enzyme inducers
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Common: Diarrhoea, fever, gastrointestinal discomfort, headache, nausea, vomiting, skin reactions, upper respiratory tract infection
- Uncommon: Akathisia, anxiety, arthralgia, asthenia
- Rare: Angioedema, concentration impaired, disorientation, erythema nodosum, hallucination, hepatic disorders
Pregnancy & Lactation
- Crosses the placenta in rats and rabbits
- Caution should be exercised during nursing
Precautions & Warnings
- Not indicated for reversal of bronchospasm in acute asthma attacks
- Appropriate rescue medication should be available
- Not to be abruptly substituted for inhaled or oral corticosteroids
- Not to be used as monotherapy for exercise induced bronchospasm
Overdose Effects
- Majority of reports showed no adverse experiences
- Usual supportive measures recommended in case of overdose
