Indications
- Prophylaxis and chronic treatment of asthma

Pharmacology
- Montelukast is a selective and orally active leukotriene receptor antagonist that inhibits the cysteinyl leukotriene receptor (CysLT1)
- Cysteinyl leukotrienes and leukotriene receptor occupation have been correlated with the pathophysiology of asthma & allergic rhinitis
Dosage & Administration
- Adult dosage: Montelukast 10 mg tablet once daily
- Pediatric dosage (6 to 14 years): Montelukast 5 mg tablet once daily
- Pediatric dosage (2 to 5 years): Montelukast 4 mg tablet once daily
- Pediatric dosage (6 months to 5 years): Montelukast 4 mg oral granules once daily
- No dosage adjustment required for mild-to-moderate hepatic insufficiency or renal insufficiency
- Elderly: No dosage adjustment required
Interaction
- No clinically important effects on the pharmacokinetics of theophylline, prednisone, prednisolone, oral contraceptives, terfenadine, digoxin, and warfarin
- Safety with a wide range of commonly prescribed drugs observed in clinical studies
Contraindications
- Montelukast is contraindicated in patients who are hypersensitive to any component of this product
Side Effects
- Common side effects: Diarrhoea, fever, gastrointestinal discomfort, headache, nausea, vomiting, skin reactions, upper respiratory tract infection
- Uncommon side effects: Akathisia, anxiety, arthralgia, asthenia, abnormal behavior, depression, dizziness, drowsiness, dry mouth, haemorrhage, irritability
- Rare side effects: Angioedema, concentration impaired, disorientation, eosinophilic granulomatosis with polyangiitis, erythema nodosum, hallucination, hepatic disorders
Pregnancy & Lactation
- Montelukast crosses the placenta in rats and rabbits
- Caution should be exercised when given to nursing mothers
Precautions & Warnings
- Not indicated for use in the reversal of bronchospasm in acute asthma attacks
- Appropriate rescue medication should be available
- Should not be abruptly substituted for inhaled or oral corticosteroids
- Not to be used as monotherapy for the treatment of exercise-induced bronchospasm
- Avoidance in patients with known aspirin sensitivity
- Caution in aspirin-sensitive asthmatic patients
Overdose Effects
- No adverse experiences in the majority of overdosage reports
- Usual supportive measures should be employed in the event of overdose
