Indications

  • Prophylaxis and chronic treatment of asthma
  • Acute prevention of Exercise-Induced Bronchoconstriction (EIB)
  • Relief of symptoms of Allergic Rhinitis (AR): Seasonal & Perennial Allergic Rhinitis

Pharmacology

  • Montelukast is a selective and orally active leukotriene receptor antagonist that inhibits the cysteinyl leukotriene receptor (CysLT1)
  • Cysteinyl leukotrienes and leukotriene receptor occupation have been correlated with the pathophysiology of asthma & allergic rhinitis

Dosage & Administration

  • Adult dosage: Montelukast 10 mg tablet once daily
  • Pediatric dosage (6 to 14 years): Montelukast 5 mg tablet once daily
  • Pediatric dosage (2 to 5 years): Montelukast 4 mg tablet once daily
  • Pediatric dosage (6 months to 5 years): Montelukast 4 mg oral granules once daily
  • No dosage adjustment required for mild-to-moderate hepatic insufficiency or renal insufficiency
  • Elderly: No dosage adjustment required

Interaction

  • No clinically important effects on the pharmacokinetics of theophylline, prednisone, prednisolone, oral contraceptives, terfenadine, digoxin, and warfarin
  • Safety with a wide range of commonly prescribed drugs observed in clinical studies

Contraindications

  • Montelukast is contraindicated in patients who are hypersensitive to any component of this product

Side Effects

  • Common side effects: Diarrhoea, fever, gastrointestinal discomfort, headache, nausea, vomiting, skin reactions, upper respiratory tract infection
  • Uncommon side effects: Akathisia, anxiety, arthralgia, asthenia, abnormal behavior, depression, dizziness, drowsiness, dry mouth, haemorrhage, irritability
  • Rare side effects: Angioedema, concentration impaired, disorientation, eosinophilic granulomatosis with polyangiitis, erythema nodosum, hallucination, hepatic disorders

Pregnancy & Lactation

  • Montelukast crosses the placenta in rats and rabbits
  • Caution should be exercised when given to nursing mothers

Precautions & Warnings

  • Not indicated for use in the reversal of bronchospasm in acute asthma attacks
  • Appropriate rescue medication should be available
  • Should not be abruptly substituted for inhaled or oral corticosteroids
  • Not to be used as monotherapy for the treatment of exercise-induced bronchospasm
  • Avoidance in patients with known aspirin sensitivity
  • Caution in aspirin-sensitive asthmatic patients

Overdose Effects

  • No adverse experiences in the majority of overdosage reports
  • Usual supportive measures should be employed in the event of overdose
