Indications
- Prophylaxis and chronic treatment of asthma
- Acute prevention of Exercise-Induced Bronchoconstriction (EIB)
- Relief of symptoms of Allergic Rhinitis (AR): Seasonal & Perennial Allergic Rhinitis
Pharmacology
- Selective and orally active leukotriene receptor antagonist
- Inhibits the cysteinyl leukotriene receptor (CysLT1)
- Correlated with the pathophysiology of asthma & allergic rhinitis
Dosage & administration
- Adult dosage: Montelukast 10 mg tablet once daily
- Pediatric dosage: Montelukast 5 mg/4 mg tablet once daily
- Usable in pediatric patients with asthma 6 months to 14 years of age
- No dosage adjustment required in mild-to-moderate hepatic insufficiency
- No dosage adjustment recommended in patients with renal insufficiency
- Similar pharmacokinetic profile in elderly and younger adults
Interaction
- No apparent increase in adverse reactions when administered with other therapies
- No clinically important effects on the pharmacokinetics of recommended drugs
- Used concomitantly with a wide range of commonly prescribed drugs in clinical studies
- Phenobarbital decreased the AUC of Molukat by approximately 40%
Contraindications
- Contraindicated in patients hypersensitive to any component
Side effects
- Common side effects: Diarrhea, fever, headache, nausea, vomiting
- Uncommon side effects: Anxiety, depression, dizziness, dry mouth, sleep disorders
- Rare side effects: Angioedema, hallucination, memory loss, palpitations, tremor
Pregnancy & lactation
- Crosses the placenta following oral dosing in rats and rabbits
- Use during pregnancy only if clearly needed
- Caution should be exercised when given to a nursing mother
Precautions & warnings
- Not indicated for use in the reversal of bronchospasm in acute asthma attacks
- Appropriate rescue medication should be available
- Therapy can be continued during acute exacerbations of asthma
- Molukat should not be abruptly substituted for inhaled or oral corticosteroids
- Not to be used as monotherapy for the treatment and management of exercise induced bronchospasm
- Patients with known aspirin sensitivity should continue avoidance of aspirin or non-steroidal anti-inflammatory agents
Overdose effects
- No adverse experiences in the majority of overdosage reports
- Abdominal pain, somnolence, thirst, headache, vomiting, psychomotor hyperactivity
