Indications

  • Prophylaxis and chronic treatment of asthma
  • Acute prevention of Exercise-Induced Bronchoconstriction (EIB)
  • Relief of symptoms of Allergic Rhinitis (AR): Seasonal & Perennial Allergic Rhinitis

Pharmacology

  • Selective and orally active leukotriene receptor antagonist
  • Inhibition of the cysteinyl leukotriene receptor (CysLT1)
  • Correlation with asthma & allergic rhinitis pathophysiology

Dosage & Administration

  • Adult dosage: Montelukast 10 mg tablet once daily
  • Pediatric dosage: Montelukast 5 mg for children aged 6-14 years
  • Pediatric dosage: Montelukast 4 mg for children aged 2-5 years
  • Pediatric dosage: Montelukast 4 mg oral granules for children aged 6 months to 5 years

Interactions

  • No clinically important effects on theophylline, prednisone, terfenadine, digoxin
  • Used concomitantly with a wide range of commonly prescribed drugs
  • Effect on drugs metabolized by cytochrome P450 enzymes

Contraindications

  • Hypersensitivity to any component

Side Effects

  • Common: Diarrhoea, headache, nausea, vomiting
  • Uncommon: Anxiety, dizziness, malaise, seizure
  • Rare: Angioedema, hallucination, palpitations, suicidal tendencies

Pregnancy & Lactation

  • Crosses placenta in rats and rabbits
  • Use during pregnancy only if clearly needed
  • Caution in nursing mothers

Precautions & Warnings

  • Not for reversal of bronchospasm in acute asthma attacks
  • Appropriate rescue medication advised
  • Caution in patients with known aspirin sensitivity

Overdose Effects

  • Adverse experiences: abdominal pain, somnolence, vomiting
  • Usual supportive measures in case of overdose
