Indications
- Short-term administration for rheumatic disorders

Pharmacology
- White to cream-colored crystalline powder
- Practically insoluble in water
- Very soluble in alcohol
- Synthetic glucocorticoid with anti-inflammatory action
- Extended duration of effect
- Adrenal suppression occurs within 24 to 48 hours
- Returns to normal in 30 to 40 days
Dosage & Administration
- Initial dose may vary from 2.5 mg to 100 mg per day
- Recommended initial dose for systemic therapy: 60 mg
- Injection should be made deeply into the gluteal muscle
- Single local injection is frequently sufficient for intra-articular administration
- Similarly, single injections into several joints, up to a total of 80 mg, have been given
Interaction
- Aminoglutethimide may lead to loss of corticosteroid-induced adrenal suppression
- Concomitant use with amphotericin B or potassium-depleting agents may lead to hypokalemia
- Macrolide antibiotics may decrease corticosteroid clearance
Contraindications
- Hypersensitivity to any components of the product
- Contraindicated for idiopathic thrombocytopenic purpura
Side Effects
- Allergic reactions including anaphylaxis and angioedema
- Cardiovascular issues like bradycardia and hypertension
- Dermatologic reactions including acne and rash
- Endocrine issues such as glycosuria and menstrual irregularities
- Fluid and electrolyte disturbances
- Gastrointestinal issues including hepatomegaly and peptic ulcer
- Musculoskeletal problems like osteoporosis and muscle weakness
- Neurological and psychiatric effects including depression and insomnia
- Ophthalmic effects like increased intraocular pressure and cataracts
Pregnancy & Lactation
- Pregnancy Category C
- Systemically administered corticosteroids appear in human milk
- Caution should be exercised when administered to a nursing woman
Precautions & Warnings
- Steroids should be used with caution in active or latent peptic ulcers
- Intra-articularly injected corticosteroids may be systemically absorbed
- Corticosteroids decrease bone formation and increase bone resorption
- Psychic derangements may appear when corticosteroids are used
- Intraocular pressure should be monitored if steroid therapy is continued for more than 6 weeks
Use in Special Populations
- Premature and low-birth-weight infants may be more likely to develop toxicity
- No overall differences in safety or effectiveness were observed between elderly and younger subjects
Overdose Effects
- Treatment is by supportive and symptomatic therapy
