Pharmacology
- White to cream-colored crystalline powder
- Practically insoluble in water
- Very soluble in alcohol
- Synthetic glucocorticoid corticosteroid
- Suitable for intramuscular and intra-articular use
- Extended duration of effect
- Adrenal suppression occurs within 24 to 48 hours
- Returns to normal in 30 to 40 days
Dosage & Administration
- Initial dose may vary from 2.5 mg to 100 mg per day
- Strict aseptic technique mandatory
- Suggested initial dose for systemic therapy is 60 mg injected deeply into the gluteal muscle
- Single local injection of 2.5 mg to 15 mg for larger joints
- Shake vial before use to ensure a uniform suspension
Interaction
- Aminoglutethimide may lead to loss of corticosteroid-induced adrenal suppression
- Amphotericin B injection and potassium-depleting agents may cause hypokalemia
- Macrolide antibiotics could decrease corticosteroid clearance
- Anticholinesterases and corticosteroids may produce severe weakness in myasthenia gravis patients
- Coadministration of corticosteroids and warfarin usually results in inhibition of response to warfarin
Contraindications
- Hypersensitivity to any components of the product
- Contraindicated for idiopathic thrombocytopenic purpura

Pregnancy & Lactation
- Pregnancy Category C
- Corticosteroids shown to be teratogenic in many species
- Corticosteroids appear in human milk and could suppress growth or cause other untoward effects
Precautions & Warnings
- Trimcort sensitive to heat and should not be autoclaved
- Lowest possible dose should be used to control the condition
- Caution in patients with congestive heart failure, hypertension or renal insufficiency
- Intra-articularly injected corticosteroids may be systemically absorbed
- Steroids should be used with caution in various gastrointestinal conditions
Use in Special Populations
- Premature and low-birth-weight infants more likely to develop toxicity
- No overall differences in safety or effectiveness observed between elderly and younger subjects
Overdose Effects
- Treatment of acute overdosage is by supportive and symptomatic therapy
- For chronic overdosage in severe disease requiring continuous therapy, the dosage may be temporarily reduced or alternate day treatment introduced
