Indications
- Maintenance treatment of severe chronic obstructive pulmonary disease (COPD)
- Add on to bronchodilator treatment
Pharmacology
- Phosphodiesterase-4 (PDE-4) inhibitor
- Anti-inflammatory effects
- Antimodulatory effects
- Increased levels of intracellular cyclic AMP
Dosage & Administration
- Recommended dose: 500 micrograms Roflumilast once daily
- Study duration: up to one year
- Swallow with water
- Take at the same time every day
- Can be taken with or without food
- No dose adjustment for elderly or renal impairment
- Caution in patients with mild hepatic impairment
- Not recommended for pediatric use
Interaction
- No proven cases of hazardous interactions
- Reported cases of interaction with ethanol, pyrimethamine, dapsone, and potentiation of betahistine with salbutamol
Contraindications
- Hypersensitivity to Roflumilast or excipients
- Moderate or severe hepatic impairment
Side Effects
- Adverse reactions: diarrhea, weight decreased, nausea, abdominal pain, headache
- Occurrence within first weeks of therapy
- Resolved on continued treatment
- Various body system disorders and frequencies
Pregnancy & Lactation
- Not recommended during pregnancy or in women of childbearing potential not using contraception
- Crosses placenta in pregnant rats
- Excretion in milk
- Not recommended during breastfeeding
Precautions & Warnings
- Inform patients about risks and precautions
- Not indicated as rescue medicinal product for acute bronchospasms
- Monitoring of body weight
- Caution in patients with specific clinical conditions and psychiatric disorders
- Reassessment of treatment in case of persistent intolerability
- Not recommended concomitant treatment with theophylline
- Lactose content - caution in patients with rare hereditary problems
Overdose Effects
- Symptoms observed at increased doses in Phase I studies
- Recommended appropriate supportive medical care
- Haemodialysis not likely to be efficient
- Dialysability by peritoneal dialysis unknown
